It is easy to make a referral to Young Foundations. Just give us a call on 01325 366365 and we can discuss individual cases with you.

Once a potential referral is highlighted, contact will be made with the Young Foundations' placement team. All relevant information will then be passed on to the appropriate Homes Manager for further discussion and for the admission criteria to be examined. If a home is seen as appropriate and agreed, a Service Agreement will then be forwarded to the Local Authority for authorisation.

Arrangements for admission can then only be made for the young person to be placed if the following information is available:

• Medical consent
• Care Plan
• Essential Information Part 1

Pre-placement meetings are always helpful in all cases other than an emergency.

Provided the relevant information is available, a placement decision can be made within hours. A service agreement needs to be signed to facilitate the placement and if necessary pre-placement visits and meetings can be arranged.
