Once a young person is welcomed into a Young Foundations
home they will have a key worker allocated to them. An independent
visitor will be selected at the earliest opportunity where appropriate.
An assessment will take place to identify the young person's individual
needs. This assessment will be multi-faceted and multi-disciplined
depending on the strengths and difficulties the young person is
experiencing, and where they are being referred from. Assessment
tools will be used where appropriate to act as a baseline to track
each individual's development.
The placement plans will incorporate boundaries and objectives;
they will follow a schedule designed to bring stability into their
lives.
The placement plan will include:
- Health needs and health promotion
- Care needs, including safeguarding and promoting the welfare
of the child
- Physical and emotional needs
- Educational needs and attainment targets
- Cultural, religious, language and racial needs and how they
will be met
- Leisure needs
- Contact arrangements with family, friends and significant other
people as appropriate
