Transition Care

Young Foundations Transition Care is a new service targeted at young people who are betwixt children’s care services and adult services.

Transition Care can be summarised as follows:

  • Specialist residential care for more vulnerable young people 16 to 25 and those capable of entering further education
  • For young people with enduring and complex needs, who may find difficulty in finding appropriate placements
  • Flexible and tailored care programmes, supported by skilled teachers and specialist staff
  • Access to our multi-disciplinary team clinical support
  • Regular measuring of progress and outcomes
  • Flexible in coping with individual needs, including learning disability, autistic spectrum disorders, complex needs and challenging behaviour
  • A unique environment and care pathway tailored for young people who may eventually step up to adult services, and for those stepping down towards community-based living
